Step 1: Prepare Cover Page
Print the Cover Page and fill in the information.
 
Step 2: Write Proposal
Using our Proposal form as a guide, provide background and project descriptions.
Tip: Be concise.
 
Step 3: Gather Attachments
Using our Attachments Checklist as a guide, gather required documentation and (optional) supporting materials.
Tip: Print the checklist to help you organize.
 

Step 4: Mail Packet
Your application packet consists of:

  1. The signed and dated cover page
  2. The proposal summary
  3. The attachments
  4. Any supporting material that is relevant to your request
